**All fields in RED must be filled out
1. Select a LOCATION where your office is located ==> Campus/Facility, Building, and Area
2. Fill out REQUESTOR INFORMATION ==> Full Name, Phone #, TCC Email, and Department
3. Fill out REQUEST ACTION ==> request description, justification, date and time area available
4. Answer questions appropriately pertaining to your request

Note: Drug Test and Defensive Driving Training must be completed in order to be an authorized TCC Vehicle Driver and/or Equipment Operator. For questions, call 817-515-9000.
